Patient rights
When using the services of QMEDIC sp. z o.o. in the HelpDoc service, you have all the rights provided for in the Act of 6 November 2008 on Patient Rights and the Patient Ombudsman (ustawa z dnia 6 listopada 2008 r. o prawach pacjenta i Rzeczniku Praw Pacjenta). Below we describe them briefly and explain how to exercise them.
1. Right to healthcare services
You have the right to services that reflect current medical knowledge, provided with due diligence by persons with appropriate qualifications. If your state of health requires a physical examination, the physician will tell you so and indicate where to seek in-person help.
In an emergency that threatens your life or health, do not use the service. Call the emergency number 112 or 999.
2. Right to information
You have the right to clear information about your state of health, diagnosis, proposed treatment methods, their consequences and prognosis. You may name a person to whom the physician may give this information, or decline to receive it.
3. Right to consent
A healthcare service requires your consent. Placing an order or starting a consultation constitutes consent to a service provided remotely. You may withdraw it at any time. For a patient under 16, consent is given by the statutory representative, and a patient aged between 16 and 18 gives it together with the representative.
4. Right to confidentiality
Information about you and your health is confidential. Physicians and persons authorised by QMEDIC keep it confidential also after the treatment has ended, subject to the exceptions provided for in the Act.
5. Right to respect for privacy and dignity
Consultations take place with respect for your privacy and dignity. The physician conducts the conversation in conditions that ensure confidentiality.
6. Right to medical records
You have the right to access your medical records. You may receive them in electronic form, as a copy or as an extract. Submit your request to [email protected]. QMEDIC makes the records available free of charge, in the manner described in the Organisational Rules.
7. Right to object to a physician's opinion or ruling
If a physician's opinion or ruling affects your rights or obligations under the law, you may lodge an objection with the Medical Commission (Komisja Lekarska) at the Patient Ombudsman (Rzecznik Praw Pacjenta) within 30 days of its issue.
8. Right to report adverse drug reactions
You may report an adverse reaction to a medicine to a physician, a pharmacist or directly to the Office for Registration of Medicinal Products, Medical Devices and Biocidal Products (Urząd Rejestracji Produktów Leczniczych, Wyrobów Medycznych i Produktów Biobójczych).
9. Complaints and assistance
